owned subsidiary of US Risk Insurance Groups. Lighthouse is a program
underwriting company with three proprietary programs.
Founded in 1994, Lighthouse was acquired by US Risk in 2006.
Our three proprietary programs include:

TAKE 1 is an all lines program for the
entertainment and presentation Industries. TAKE 1 is underwritten
in our Santa Ana, CA office and managed by Scott Carol.
FFIC is the carrier for the TAKE 1 program. Our inland marine form
is one of the broadest in the insurance industry. |

|
StaffPak is our program for the Staffing Industry and
includes all lines but WC for temporary staffing companies and PEOs.
Our carrier, Arch Insurance, has partnered with us to provide one of
the most comprehensine staffing programs in the country. We can write
on both an admitted and non-admitted basis. Our professional liability
is written on an occurrence basis. StaffPak is underwritten in Dallas
and managed by Peter Stanislaw. |

The first program specifically designed for assisted living.
We underwrite in all lines except WC program. We can write property and auto
unsupported. Our product provides emergency evacuation and media defense cover
and is competitively priced. ElderCare is underwritten in Boston and is managed
by Kevin Mordarski. |
