A "Complete Submission" consists of:
- Completed Take1 Supplemental Application
- Completed, signed, and dated ACORD Application
- List of Equipment (include Values and Serial numbers)
- Copy of Rental Contract/Agreement
- Hard Copy of Loss Runs
- Applicable Brochures and Website Address
- Financials (most recent)
