It is important that we receive complete underwriting information in order to provide a timely quote.

A "Complete Submission" for D&O consists of:

  • Completed, Signed and Dated D&O Application
  • Most recent annual financial report (IRS 990 or Annual Audit)


A "Complete Submission" for the Package consists of:

  • Acord Applications
  • Supplemental Applications (contact Andy Cooley as applications vary based on need)
  • Loss Runs
  • Financials
  • Brochure/Website Information
  • Copy of any licenses held by insured
