in order to provide a timely quote.
A "Complete Submission" for D&O consists of:
- Completed, Signed and Dated D&O Application
- Most recent annual financial report (IRS 990 or Annual Audit)
A "Complete Submission" for the Package consists of:
- Acord Applications
- Supplemental Applications (contact Andy Cooley as applications vary based on need)
- Loss Runs
- Financials
- Brochure/Website Information
- Copy of any licenses held by insured
